We utilize the Centers for Disease Control's approved curriculum, Prevent T2, to help prevent (or delay) the onset of type 2 diabetes for those with prediabetes.
We have partnered with TMF and the Center for Medicare Services to offer a 6-week program to Medicare recipients living with type 2 diabetes.
We offer one-on-one coaching for those who are unable to attend group sessions. There will be an initial evaluation to access needs and how we can assist the individual.
